The Canterbury District Health Board will commission an external investigation into the tragic death of Coalgate man, Dean Carroll, 25, who died on Saturday 14 April.
The Coroner is also investigating the death of Mr Carroll, who had visited Christchurch Hospital’s Emergency Department the previous afternoon.
The CDHB is inviting an external investigation in order to find out exactly what occurred and make any necessary changes at the hospital.
“Mr Carroll’s death was a tragedy. Our sincere condolences go out to his family knowing that anything we say or do at this time cannot ease their pain or sense of loss. We will continue to stay in close contact with them throughout the investigations,” Mark Leggett, GM Medical and Surgical Services said.
A meeting between Mr Carroll’s family, CDHB clinicians and managers will take place soon to talk through the next steps that will be taken in the investigation and any other issues of concern for the family.
