The Canterbury DHB Contracted Provider Quality Network Group’s purpose is to
facilitate networking and sharing of information amongst Contracted Providers
Quality Practitioners. The Group is held on a monthly basis and provides a forum
for sharing quality information, standards, national strategies and trends and
provides a mechanism for disseminating quality information.
Membership is open to quality practitioners belonging to Canterbury DHB
Contracted Providers who have an interest in quality management practices within
their organisations. The Group provides a mechanism for input into Ministry of
Health and Canterbury DHB policy and processes and provides the opportunity for
guidance and mentoring. The Group also provides the opportunity to develop and
share quality initiatives amongst the members and their subsequent
organisations.
An incident form has been developed for the community providers to record any issues/problems associated with the transfer of care from the hospital to the community provider. The initiative aims to facilitate better communication between providers and hospitals. Completed forms are to be faxed to Clare Keast (Department of Nursing, Christchurch Hospital). Clare is responsible for following up and investigating the incident and liaise with the person who has reported the incident. This includes the outcome of the investigation.
Canterbury DHB Contracted Provider Organisations are encouraged to enter the Canterbury DHB Quality Improvement and Innovation Awards which are sponsored by the Canterbury DHB Quality and Patient Safety Council.
